Initial Comments:
An onsite follow-up survey conducted on August 18, 2021 found that Dialysis Clinic, Inc. had corrected the following condition and standard level deficiencies cited under 42 CFR, Part 494, Subparts A, B, C, and D, Conditions for Coverage of Suppliers of End-Stage Renal Disease (ESRD) Services. The deficiencies were cited as a result of a Medicare recertification survey that was completed on July 22, 2021.
As a result of the survey, the condition level citation has been lifted.
